Independent Living
Our independent living residences are for those who wish to maintain a fulfilling and rewarding life without the demanding responsibilities of maintaining a home -- and with the assurance that additional assistance is available, if it is ever needed.
If needed, Pueblo Norte offers the wonderful benefit of home health. Our Independent Living residents needing a bit of extra support, receive it while remain in their own apartment or villa home, thanks to our partnership with Carefree Home Health, which is on-site at Pueblo Norte.
Also, independent living residents can opt for our Lifecare program. Should a need arise for assisted living or skilled nursing care, residents who are covered under our Lifecare program pay the same monthly fee for these services charged for their independent living home, with only an additional charge for extra meals and ancillary items. Leasing is also an option.
Assisted Living
Assisted Living at Pueblo Norte is designed for seniors who want residential living with some level of supervision and care. With our on-site, 7-day-per-week caregivers, our residents have access to caring and skilled people ready to assist as necessary. Additionally, all residents are provided with emergency call pendants, providing the added security of staff response throughout the campus.
Individualized assistance provided by our caring staff allows our residents to maximize their independence and participate in activities that promote socialization, friendships and staying active.
With our on-site, seven-day per week caregivers, our residents have access to caring and skilled people ready to assist as necessary.
Additionally, all residents are provided with emergency call pendants, providing the added security of staff response throughout the campus.
Skilled Nursing Care
Pueblo Norte provides skilled nursing services in a licensed and Medicare-certified setting that includes both private and semi-private accommodations.
Our trained and licensed staff understand that the choice of a skilled nursing facility usually takes place during a difficult period in a resident’s life. Our experienced admissions staff help residents and their families with this process and transition.
Our approach to care involves the active participation of residents, their families and physicians with the care team at Pueblo Norte. Our goal is to develop individualized plans of care designed to meet the medical, emotional, spiritual, social, and activity needs of each resident.
Rehabilitation
Both inpatient and outpatient rehabilitation services are available on-site at Pueblo Norte. Therapy services can be vital to an individual’s recovery.
Pueblo Norte offers physical, occupational, and speech therapy; these services are covered by Medicare and many Managed Care programs.
Our team of therapists, nurses, social service and home health professionals review and assess each individual’s rehabilitation program, and tailor it to meet his or her needs.
